Integrated Health Home Benefits Medicaid Recipients in Iowa

An Integrated Health Home (IHH) is comprised of professionals who work together to provide patient-centered, coordinated care for adults with a serious mental illness and for children with a serious emotional disturbance. According to the Iowa Department of Human Services, the IHH program will be administered by Medicaid Managed Care Organizations (MCOs), and services will be provided by community-based IHH agencies.

"Integrated Health Home is a team that will assist with your mental, physical, and social needs," explained IHH Director Nicole McCoy. "There is a process to follow in order to determine eligibility for a member. You must have Medicaid and a significant mental health diagnosis, and you must be determined as eligible by the member's MCO." An MCO is an affiliated insurance company.

In 2016, the Iowa DHS transitioned most Iowa Medicaid members to IA Health Link, a managed care program that is administered by three contracted MCOs, which provide members with comprehensive health care services, including physical, behavioral, and long-term care services and support. The MCOs have a comprehensive network of health care providers that ensures quality care is administered to all Medicaid members.

Capstone Behavioral Health Care Center is the agency that houses the IHH that serves Jasper, Poweshiek, and Marion counties. Capstone employs the IHH staff; a therapist or medication prescriber at Capstone may also make the initial referral to connect a patient with IHH services.

IHH provides Care Coordination and Peer Support to all members. "Care Coordinators work with the members to monitor patients' needs and connect them to services and providers as needed," said McCoy. "Peer Supports provide support in the area of mental health, and assist with the journey towards recovery and learning to live well with a mental health diagnosis."
