Jay Allen Heath, 44, of Prairie City, died Saturday, April 23, 2016, at his home in Prairie City. A memorial service will be at 10:30 a.m. Thursday, April 28 at the Ira United Church, 6834 Main St. in Ira. The family will greet friends immediately following the memorial service at the church with a lunch. Memorials for Jay’s sons may be left at the Wallace Family Funeral Home & Crematory or at the church on the morning of the service. Memorials may also be mailed to the funeral home, please add Attn: Heath Family on the envelope.
Those left to honor Jay’s memory are his wife, Kristie Heath of Monroe; sons, Jonathon, Christopher, Benjamin and Zachary Heath, all of Monroe; his mother, Jackie Heath of Mingo; stepsiblings, John (Janet) Heath of Winterset, Joan (Tim) Rasmussen of Omaha, Neb., and Julie (Jeff) Menssen of Dunnell, Minn.
He was preceded in death by his father, John C. Heath.
